Tatsuhiro Tsujikawa 3a95bf47f3 lib: Consistent conditional macro comments
Make conditional macro comments consistent.

- Repeat condition in closing #endif.
- Use #ifdef for a single macro.  Do not use #if defined(...) in this
  case.  Use defined(...) form when repeating condition in #endif.
- Apply De Morgan when negating conditions in #else.
